Southeast Health Center - 2401 Keith Street, San Francisco, CA 94124

  • For residents ages 65+ living in 94124 and 94134

  • Will qualify regardless of insurance

  • Proof of age and residence, such as a utility bill or driver’s license is required

  • No appointment is required

  • Vaccine supply remains low and you may be asked to return the following day if the site reaches capacity

Zuckerberg San Francisco General Hospital - Learning Center – 1001 Potrero Avenue, Building 30 on the 2nd floor (off 23rd Street), San Francisco, CA 94110

  • For residents ages 65+ living in 94110, 94112, 94107 with Medi-Cal

  • Proof of age and residence such as a utility bill or driver’s license is required

  • Medi-Cal patients must bring insurance card

  • No appointment is required

  • Vaccine supply remains low and you may be asked to return the following day if the site reaches capacity

Bayview Child Health Center 3450 3rd Street, Building 2, San Francisco, CA 94124

  • Please call to make an appointment before visiting the site.

  • Need to be age 65 +, healthcare worker, or a long term care resident

  • Must call (415) 890 - 2409 and leave a voicemail with your full name, date of birth, phone number, and which of the categories above you fit into.

  • A nurse will call back within 48 hours to schedule for an appointment
